you don't have a dentist in here exactly; maybe the frame narrative should be about a child who doesn't want to go to the dentist, but has a dream in which his mouth is a building-site populated by creatures, germs on pogo-sticks jumping up and down on gums and roots, teeth being exploded like derelict buildings etc,, and then, the child does want to go to the dentist. It feels as if waiting to go the dentist or the prospect of going might be the trigger for the fantastical sequence?
